Fields with a (*) is a required fields. 

Passwords have to be in 6 characters long (Example: "cat954")

After all the information's are entered, click on the SUBMIT button to submit your registration.

Click on the RESET button if you want to clear all the information and re-type the information again.

Once the registration is successful, you will receive the "REGISTRATION SUCCESSFUL" screen where your MEMBERSHIP ID number will be given. Copy down, or remember the id number and your password, which you will need to login with.
